There was an incident there years ago where it was reported that a Gunner jokingly fired the main gun at one of his buddies thinking that the tube was clear. It was not. You would think that the fire control system would warn that the tube is not clear with a round in the chamber.
I hope nobody takes my post the wrong way. I have the utmost respect for all the armed forces.

I can't speak for tank drills, but Field Artillery cannon drills require a report of "bore clear" upon completion of firing. Note. This requires a visual inspection of the cannon bore to ensure it is in fact clear.
