Fentanyl is a fantastic analgesic for acute pain. Used it a LOT in the field. Measured in micrograms, administered via slow IV push vs. "slam it in." 25 mcg was a good starting dose. 200 was the most I ever gave, and that was four doses of 50 mcs.

I'm having one custom made - Lone Star Holsters. I'm using the AC extended TDL. If it's Kydex, it's a simple matter of heat and slight re-molding (push the gun in and out several times). If it's leather, that's a different story.
