Why weren't the de-icer on the plane working ir activated if that's the case! Not all have them given the altitude flown at. Look for a usually black strip on the leading edge of the wing as it inflates enough to break it. Might be other ways than that in the last 40 years since my dad flew?They’re speculating pilot disorientation, but icing sounds plausible.
