Kind of at a loss here so hoping someone has some ideas.

About a week ago I was cleaning my hellion and decided to deep clean the BCG. I took it apart, cleaned it, lubed it, put it back together and did a functions check and everything seemed good.

I went to the range a few days ago and the first issue I noticed was some trouble getting magazines to lock in. I am using the lunartech flared magwell which may have caused this specific issue. After taking out a round from each magazine they were able to latch properly.

That's when the trouble started. I'd be able to get off one or two shots before pulling the trigger and having nothing happened. Put the gun on safety and went to check chamber and I'd see 3 rounds, one casing that wasn't ejected and 2 live rounds that were bent to hell and back. This happened two times before I decided to stop firing the gun.

Any ideas on what could have caused this? I'm pretty sure I put the BCG back together properly. My first thought was the flared magwell but I'm not sure how it could contribute to a triple feed.

While I cannot give you a definitive answer, I have an observation. Ensure your bolt/extractor and cam key are oriented to the proper ejection side. I wonder if the failure to extract may have been caused by the extractor on the wrong side with the cam key angled improperly. This design is easy to screw up during re-assembly after cleaning...I still need the manual after owning for a year. But I never claimed to be very smart. Don't think if this were the case it would cause pulling 2 live rounds from the mag instead of just 1 however. That might be a different issue.

My Hellion is very ammo-sensitive and does not like any .223 I've fed it...does not extract. Runs all 5.56 Nato, just no .223 (I admit I've only fed cheap .223 brass-case Wolf and Winchester before discovering this and settling on brand-name brass 5.56Nato.) Good luck, there are many others here who may be more help than I.

Now that I re-read and process what I typed above, It's possible you only have 1 issue...double-feeding. If your rifle fails to extract .223 like mine, try 5.56 Nato. The bolt stripping more than one round might be the only issue that is mag-related.
 
I'm not sure why you can't insert a fully loaded mag to start with. I can with mine. Between not being able to load a full mag and the double stripping cartridges it almost sounds like the mag isn't in the right posture when inserted. Not sure what the lunartech flared Maxwell is but I'd go back to all original parts to start with.

Definitely pull out the manual and confirm your bolt/extractor configuration for whichever side you are using.

I use all Frontier 5.56 (bought a few cases on sale) ammo in mine and never had a failure. I've never tried any .223 in it but remember trying it in my AR15 and it wasn't found of it. So I stick with the Frontier 5.56, both 55 & 62 grain work well in both rifles.

There are few coincidences in such instances, generally there is cause and effect. Most likely something you did caused the issue.

Going back over it step by step may determine the issue and the fix.
