Mag issue??

Out shooting today and had this happen. I only have 200 rounds through the gun, which was brought new last month. This is the second time this has happened. So, I tried my mag in the wife's gun, and had this weird double feed (wife's gun has the custom grips). In both instances the slide was locked back and I had to drop the mag to get the slide to move. Anyone have any thoughts? I did call SA and they are sending me a new mag.

It is not extracting-could be a loose extractor-as in not enough tension.
The empty is still in the chamber when the slide comes back and tries to load a new round behind it
Thats not an empty in the chamber. It fired a round, brought a new one up when it extracted the spent casing and chambered it. Then the slide stayed locked back and that second round was just rolling around in there. Also, this is the wife's gun which has had no issues. It is my mag in it.

You are using the factory mags I presume since SA is sending you a new one? Do you have any other mags you can try? When you load them they are fully seated in the mag?
 
When is this happening? Once you initially load the magazine or just intermittently. Are you using the slide stop to put the pistol in battery or are you releasing it by hand? If so are you riding the slide or letting it “slingshot”?
It happens on the first round, and then intermittently. When I load the magazine, the slide is forward. After the mag is loaded, I operate the slide by hand.

Ok, are you riding the slide by chance or letting it “slingshot” I have had several buddies ride the slide and have the rounds jam up similar to what you have going on. Not necessarily saying this is the issue. Once loaded grab the slide pull it fully rearward and let it sling foward. I’m sure you already know this right now from the pictures it’s all that is coming to my mind. Maybe someone else will jump in. Also if you have any other mags defiantly try em out.

Honestly, I am not sure Part of me wants to say the I let it go. Part of me wants to say I hold onto it.
 
That’s weird, if I can ask, are you holding the gun with your thumb to close to the slide stop, inadvertently pushing the stop up under recoil?
Left handed shooter, left thumb sits on top of safety, right thumb rests on slide stop, just below the slide I guess it is very possible that my right thumb is sliding up and hitting the slide. But wouldn't I feel that happening, like a sore thumb?

Not really, I used to have that habit of a high hold on a 1911, anyway Springfield is sending you a new magazine, hope you get it figured out.

I also had some problems - 1st 300 rounds had about 10 to 12 FTF and 20 or so FTE ,called Springfield and they said run 200 rounds more - to which i had same problem - They said send it back -took about a month before i got it back - Now -- 4,000 rounds later =Not 1 problem -American eagle 147,winchester 147,blazer brass 147 ,lawman 147,sig sauer 147 - all ran flawlessly -Being new to 1911 and asking guys at the range about the problem they all pointed to the mag and all were very supportive of Springfield and backed their Quality, Guess i got a fluke

I bought this gun because of the quality. I did a lot of research on the interwebs and this issue seemed to be related to the mag. I had also read that sometimes a lot of rounds needed to ran through the gun. My wife has the same gun and has not experienced a single problem. So maybe I got a fluke also? Did SA tell you what the issue was with yours after you sent it back? SA was awesome and didn't even hesitate in sending a new mag.
