In PA it's done by the sheriff in each county. My county sheriff is extreme pro 2A. He believes everyone should have a CCW, own at least 1 handgun, if not more, and practice. The process is a single page form that they mail well in advance of your expiring ccw.
You walk into the courthouse, their office is the very first office, he moved it there after he was elected probably 20 years ago. Hand the clerk your form & $20, hangout and BS with the sheriff for a few seconds while they do an instantaneous check, snap your new photo and printout your new permit.
The whole process would go even faster if the sheriff wasn't talking with everyone that comes in. Lol!
He even goes with their equipment and staff to libraries in the county on some Saturdays. They setup on the sidewalks just outside and does renewals and greets everyone and talks to people that don't have a CCW about the benefits getting one. I know a few people that have their CCW but still have never bought a gun.
